Hemosiderin in Chronic Kidney Disease and its Role in Aggressive Hemostasis in Dogs

Hemosiderin is found in the renal tubule and is then shed into the glomerulus. This study sought to determine whether hemosiderin (heme iron) plays a role in chronic kidney disease and whether it correlates with erythropoietin-resistant anemia. As part of the study, serum levels of hemosiderin and hemopexin were quantified in 24 dogs suffering from CKD (chronic kidney disease) stage V. Hemosiderin is composed of two isoforms: adult hemosiderin in which all iron is (extra-alpha) globin bound in the adult hemoglobin and fetal hemosiderin in which all iron is free and only in the extra-alpha globin.
A significant positive correlation between renal hemosiderin and plasma hemopexin was observed. However, there was no correlation between renal hemosiderin and plasma hepcidin or erythropoietin-resistance. This suggests that hemosiderin, from a hematological standpoint, may play a role in the progression of CKD. However, from a pathophysiological standpoint, it is clear that hemosiderin plays a distinct and important role in the etiology of CKD, and future studies are warranted to determine if this is a beneficial or destructive relationship.
The relevance of these observations was examined by quantifying hemosiderin in 20 dogs with CKD stage V undergoing preemptive dialysis. These dogs had elevated plasma hemosiderin before and after dialysis. This is most likely because of the blood loss from the hemodialysis procedure. A positive correlation between hemosiderin and erythropoietin-resistance was detected in the post-dialysis period. Hemopexin levels did not significantly differ between pre- and post-dialysis, despite the presence of hemosiderin. The remaining dogs (n = 5) with CKD stage V not undergoing preemptive dialysis were found to have significant negative correlations between hemosiderin and plasma levels of erythropoietin-resistance indices. These results were independent of hepcidin levels. This study shows that hemosiderin (and therefore hematologic iron) may play a role in the etiology of CKD, which may be regulated by a different and potentially more important mechanism
